Sometimes we could all use a little help. Let Independent Living Halton be that helping hand. Outreach Community Services provide a personal support worker to assist you in your own home. Independent Living Halton is here to empower you, so you can continue to live your life – your way.

Please confirm your eligibility and follow the instructions below.
Applicants who wish to access our outreach community services must:
- Be over the age of 16
- Possess OHIP eligibility
- Live, work or study in Halton
- Have a physical disability (for example: multiple sclerosis, amyotrophic lateral sclerosis, spina bifida, cerebral palsy, spinal cord injury)
- Have full control of decisions over personal, financial and health issues.
- Outreach services are provided for 3 hours a day, between 6:00am and 11:59pm, at your home, work or school.

Once you have downloaded the forms, you may fill them electronically, save them and send to: pmoilh@independentlivinghalton.ca
